Scalp scaling, itching, redness, broken hairs or patchy hair loss can have several causes, including inflammatory skin conditions and infections. Appearance alone may not identify the cause, so persistent, spreading or worsening symptoms should be assessed by a qualified clinician.

Can a scalp fungal infection cause hair loss?

Some fungal infections of the scalp can be associated with hair breakage or hair loss, but similar symptoms can occur for other reasons. Diagnosis may require examination and sometimes testing. Scalp fungal infection should not be diagnosed from symptoms alone. Early clinical assessment is appropriate when there are spreading patches, marked inflammation, pain, discharge or persistent hair loss.

What is tinea capitis?

Tinea capitis is a fungal infection involving the scalp and hair. It is more common in children, but adults can also be affected. Possible features include scaling, itching, broken hairs, patchy hair loss or an inflamed area. These features are not specific enough to confirm the diagnosis without assessment.

How can a scalp fungal infection spread?

Some scalp fungal infections can spread through contact with an infected person or animal, or through contaminated objects and surfaces. Until a clinician has assessed the problem, avoid sharing combs, brushes, hats, towels or pillowcases. Ask the clinician whether household members, close contacts or pets need assessment; do not use harsh disinfectants on the scalp.

How is it diagnosed and treated?

A clinician may examine the scalp and may take hair or skin material for testing. Confirmed tinea capitis usually requires prescription antifungal medicine taken by mouth; treatment choice and duration depend on the organism, the person’s age, medical history and local guidance. Topical creams alone are generally not sufficient for an infection within the hair shaft. Do not self-treat an undiagnosed scalp rash with a steroid cream, because steroids can alter or worsen some fungal infections.

Will hair grow back?

Hair may regrow after the infection is treated, particularly when inflammation has not permanently damaged follicles. Regrowth is not guaranteed in every case, and severe inflammation can lead to scarring and lasting hair loss. Persistent loss after treatment needs reassessment rather than an automatic assumption that hair-transplant surgery is appropriate.

When should you seek care?

Arrange assessment for suspected scalp ringworm, spreading patches, marked tenderness or swelling, pus or discharge, fever, or persistent hair loss. A clinician can discuss testing, treatment, infection-control steps and whether another scalp condition may be responsible.
